Which Plans Does Your Employer Offer?

This is the biggest limitation. You can't get a plan that isn't offered.

Traditionally, employers pick a plan and then let the employees know what it is. Plans are usually chosen on the basis of cost, since employees complain about anything that they have to pay themselves.

You need to make it known to your employer that you want a quality plan and are willing to pay more to get it.

The only way health care can ever improve is if healthy (and therefore profitable) enrollees make it clear that they care what the plan covers for the sick.

Even if none of the plans in your area is really adequate, the fact that people are asking about rehabilitation and mental health before they enroll will start getting back to the marketing people.
